How to complete it
It's in PDF format - so you can either fill it out on screen before you print it, or print it out and use a pen. Note that it must include your handwritten signature. No digital signatures please!
Your supporting documents
You may be asked for supporting documents, so please scan and return as requested with your form:
- PAYG employees - your payslip must be no older than 31 days, and your payment summary (group certificate) from the last financial year.
- self-employed - include last 2 years' personal tax returns and/or 2 years' company/business tax returns
- if receiving rental income - provide supporting evidence of income from your investment property (e.g. current rental statement or tenancy agreement).
